Mini Diet Questionnaire

  • Do you add more than one teaspoon of sugar to your food daily?
  • Do you add salt to your food?
  • Do you smoke more than five cigarettes per week?
  • Do you drink more than one glass of wine or beer per day?
  • Do you regularly eat foods containing preservatives / additives?
  • Does less than 1/3 of your diet consist of fruit and vegetables?

If you answered yes to four or more you diet could do with improvement.

  • Do you have a sharp hunger and good digestion?
  • Do you find it easy to skip meals and often have no real appetitie?
  • Do you have a preference for salty, sweet, bitter or sour foods?
  • Are you craving certain foods that you think you can't give up?
